An exclusionary period is a period of time after the policy is written that allows for the company to deny claims that prove to be the product of a pre-existing condition. An elimination rider is a form you must sign allowing the insurance company to not pay for any treatment that is related to a condition you had in your medical history before you purchased their policy.

If you are exempt under federal HIPAA, then the insurance company may not exclude pre-existing medical conditions for any length of time. If you are not exempt under HIPAA then the company may exclude for 24 months.

Premium rates are set according to where you live, how old you are, and your health status among other factors. These rates can increase over time as you move from one age bracket to another.
